Each guidance sheet is confined to the front and back of a single page and contains information about:

Distributing a guidance sheet at the child Health Supervision visit provides parents written reinforcement of the counseling you have given.  The document also makes it clear to parents or caregivers not in attendance at the visit just what child-rearing instructions you have given.  The parent in attendance rarely absorbs all that you say.  Often, a clinician is distracted by the agenda of the moment from imparting the broad array of information the parent needs.  Using these sheets helps make each visit an efficient parent education experience.
